What Is HIV PrEP?

Pre-exposure prophylaxis (PrEP) is a highly effective medication that helps prevent HIV infection when taken as prescribed. PrEP is designed specifically for people who do not have HIV but are at higher risk of contracting the virus.

By taking PrEP regularly, you can significantly reduce your chances of acquiring HIV. Currently, two medications are approved for PrEP use, both proven to lower the risk of HIV transmission when used consistently.

How Does PrEP Work to Prevent HIV?

The medications used for PrEP belong to a class of antiviral drugs called nucleoside/nucleotide reverse transcriptase inhibitors (NRTIs). These drugs work by blocking HIV’s ability to multiply inside the body, which helps prevent infection.

Before starting PrEP, it’s essential to visit a healthcare professional for a thorough evaluation. You must first test negative for HIV before beginning PrEP. Your doctor will also order blood tests to check your liver and kidney function, ensuring they are healthy enough for the medication.

During your PrEP treatment, it is recommended to have follow-up visits every 3 months. These visits typically include HIV testing and screening for common sexually transmitted infections (STIs) such as syphilis, herpes, chlamydia, and gonorrhea.

Starting PrEP without confirming your HIV status can lead to serious complications. If you begin PrEP while unknowingly HIV positive, you risk developing drug-resistant HIV, which can make future treatment more difficult.

Why Is Medical Guidance Important Before Starting PrEP?

Consulting a doctor before starting PrEP is crucial to receive proper counselling and personalised advice. During counseling, your healthcare provider will discuss the following:

  1. The types of PrEP medications available
  2. How to take PrEP correctly for maximum effectiveness
  3. The importance of strict medication adherence
  4. Possible side effects to monitor
  5. The need for regular medical check-ups and testing
  6. The risk of other STIs and how to reduce it

Who Should Consider Taking PrEP?

PrEP is recommended for individuals at increased risk of HIV infection, including but not limited to:

  • People with an HIV-positive sexual partner
  • Those who do not consistently use condoms
  • Individuals who have had an STI in the past six months
  • People who inject drugs and share needles
  • Those with partners who are HIV positive and inject drugs
  • Individuals prescribed post-exposure prophylaxis (PEP) multiple times

Benefits and Possible Side Effects of PrEP

Benefits:

  • Highly effective in preventing HIV
  • Convenient daily pill regimen
  • A valuable option for serodiscordant couples (where one partner is HIV positive), especially when planning pregnancy

Possible Side Effects:

Some people may experience mild side effects such as:

  • Diarrhea
  • Fatigue
  • Headache
  • Nausea

Most side effects are temporary and tend to improve over time. Your doctor will help you manage any concerns during your follow-ups.

How to Take PrEP?

1. Daily PrEP

PrEP is very effective if taken daily. Studies have shown that daily dosing of co-formulated TDF/FTC can reduce the risk of getting HIV from sex by about 99%.

HIV PrEP needs to be taken for seven days before high levels of protection are achieved for both vaginal and rectal exposure to HIV.

PrEP does not protect against other STIs. Responsible, safe sex and the use of barrier contraception should be practised.

2. On-Demand PrEP

It is recommended for cisgender men who have sex with men. A double dose (two tablets) of co-formulated TDF/FTC will be taken 2-24 hours before potential sexual exposure, followed by single doses 24 and 48 hours after the initial dose.

When potential exposure is sustained for more than 24 hours, one tablet per day should be taken until the last exposure, followed by the 2 post-exposure tablets.

Using a 2-1-1 dosage timeline—two pills between 2 and 24 hours before sex, one pill 24 hours after the first pill, and a final pill 24 hours later—PrEP is shown to give you the same protection against HIV as taking it every day.

If you’re thinking about PrEP On Demand, it’s super important to take all your doses on time. If you’re worried about missing doses, chat with your doctor or counsellor about switching to daily PrEP instead.

PrEP On Demand only protects you from HIV exposure during anal sex. If you’re into activities like chem fun (CF), a daily PrEP course will be more effective in keeping you safe.

Frequently Asked Questions About PrEP

Can I Take PrEP If My Partner Is HIV Positive and I’m HIV Negative?

Yes. PrEP (Pre-Exposure Prophylaxis) is strongly recommended for people with HIV-positive partners. If your partner has not yet started HIV treatment or hasn’t achieved an undetectable viral load, taking PrEP can reduce your risk of HIV transmission by up to 99% when taken consistently.

If your partner is on effective antiretroviral therapy (ART) and has an undetectable viral load (U=U), the risk of transmission becomes virtually zero. In this case, PrEP may not be medically necessary, but it can offer peace of mind. Always consult with a healthcare provider to make an informed decision tailored to your relationship and health.

Is PrEP the Same as PEP?

No. PrEP is preventive—you take it daily before being exposed to HIV.
PEP (Post-Exposure Prophylaxis) is emergency treatment taken within 72 hours after a potential exposure to HIV.

Both are effective, but they serve different purposes. PrEP is a long-term prevention method; PEP is used short-term after a high-risk event.

Do I Still Need to Use Condoms If I’m on PrEP?

Yes. While PrEP is highly effective against HIV, it does not protect against other STIs such as syphilis, gonorrhea, or chlamydia. Using condoms along with PrEP provides dual protection for your sexual health.

You may also consider DOXYPEP (doxycycline post-exposure prophylaxis) as an added layer of STI prevention. Combining PrEP, condoms, and DOXYPEP offers the highest level of protection.

How Long Does It Take for PrEP to Work?

  • For anal sex (e.g., among MSM): PrEP reaches optimal protection after 7 consecutive days of daily use.
  • For vaginal sex or injection drug use: protection is achieved after 21 days of daily use.

If you're using PrEP On-Demand (2-1-1 dosing), it is only recommended for anal sex and must be taken precisely to be effective.

What Are the Side Effects of PrEP?

Most people tolerate PrEP well. Temporary side effects may occur during the first few weeks and include:

  • Nausea or upset stomach
  • Headache or dizziness
  • Fatigue
  • Mild diarrhea or cramps

These usually resolve as your body adjusts. In rare cases, kidney function may be affected—routine blood tests are recommended every 3 months.

What Should I Do If I Experience Side Effects?

If side effects are mild, taking PrEP before bedtime may help reduce them. If symptoms persist longer than 4 weeks or become difficult to manage, consult your healthcare provider immediately for support or adjustment.

Does PrEP Have Long-Term Side Effects?

PrEP is considered safe for long-term use. However, your doctor will monitor kidney and liver function through regular blood tests. Severe complications are rare, and most users tolerate long-term PrEP with no issues.

Can I Take PrEP If I’m On Other Medications?

Yes, PrEP does not typically interact with most common medications. However, it’s important to inform your doctor of all medications or supplements you're taking, especially if you have chronic conditions like hypertension, diabetes, or hepatitis.

Can I Use PrEP If I Drink Alcohol or Use Recreational Drugs?

Yes. PrEP is safe to use with alcohol and recreational drugs. However, these substances can affect your adherence, which is critical to PrEP’s effectiveness. Always plan and consider using reminders or alarms.

How Can I Stay Adherent to My PrEP Routine?

  • Take your pill at the same time every day.
  • Set phone reminders or use a pillbox.
  • Keep a backup pill with you when going out.
  • Pair it with a daily habit (like brushing your teeth).

What Should I Do If I Forget a Dose?

  • If you’re on daily PrEP, just take your next dose at the usual time. Missing one dose occasionally does not significantly reduce protection.
  • Do not double-dose.
  • If you’re on PrEP On-Demand (2-1-1), strict timing is essential—missing a dose can reduce protection. If you miss one, contact your provider for guidance.

How Should I Take PrEP While Travelling?

  • Take PrEP at the same time as you would at home, regardless of time zones.
  • Pack enough medication for your entire trip, plus a few extra days in case of delays.
  • Store it in its original labelled container, and carry a doctor’s note or prescription in case customs checks your medicine.
